I think I can help identify this (I even have one....). It's a Man-Beast. I can't recall who released it, but I think it was either Citadel, or part of Ral Partha's Fiend Factory range (check for FF and a number on the underside of the base). It was released in August 1978, timed to work with White Dwarf issue 8, which had an article on the Man-Beast as a new D&D character class.
